This M4L device lets you add up to 7 notes to an incoming note and alter the velocity of those notes. You will find a tutorial on that here:

You might think "I can do this with the CHORD midi effect already" - yes but with the CHORD you are limited to one set of intervals for all incoming/route notes, with the NOTE TO CHORD device you get the chance to define every chord and intervals for every single note separately!
