This is I believe, a unique system for displaying Checklists - (check boxes), on an MS Access form.
The original development of this code was for check-lists, however I was able to add more functionality by adding text boxes (questionnaire) and combo boxes ( multi-choice).

A customer required the facility to display the Checklists on a Tab control. I've added a new Zip file "CheckList_On_TAB_1a.zip" which demonstrates this requirement.
The Customer also wanted the option to select Yes, No and N/A. To Comply with this request, I converted the check-box to a combobox and added a lookups so that the user can select from more options than provided by a basic checkbox (Yes, No and N/A.)
